Please click on the above link to access the Academy of Nutrition and Dietetics online “Nutrition Care Manual.” You will be able to log on and download the diets you need for your clients. Once you log on you will see the 5 tabs below. The username to log on is watson@homehealthfoundation.org and the password is R4ldE

NCM Tab

  • Nutrition Care

    This is the Nutritional Risk Screen home page. Select a disease or condition in the navigation on the left side of this page. Risk Screen contains the nutritional indicators or criteria (by disease or condition).

  • Normal Nutrition

    - This content is appropriate for healthy adults. The General Heathful Diet section includes information and background for professionals and standards for planning a general healthful diet.

  • Older Adult Nutrition

    - This section includes a wealth of information that can be applied for the elderly in every health care setting, as well as community settings, and balances generalized geriatric information with information from long term care standards.

- Client Ed/Diets Tab - The client education handouts in the Nutrition Care Manual (NCM) are not intended to substitute for nutrition counseling with a registered dietitian nutritionist. The information is meant to serve as a general guideline and may not meet the unique nutritional needs of individual patients. All medical professionals should consult with a registered dietitian nutritionist before providing handouts to clients or patients. Each nutrition therapy handout includes:

  • A rationale for prescribing the diet
  • A sample 1-day menu linked to the USDA Nutrient Database for Standard Reference
  • Customizable sample 1-day menu

Calculators - Select a calculator in the navigation on the left side of the page. The calculators are useful when performing a nutrition assessment or planning nutrition intervention.

Formulary - The Nutrition Care Manual (NCM) Formulary contains the nutritional information about various types of oral and tube feeding nutritional solutions. This type of information is usually available in printed manufacturer's product guides and on package food labels. The NCM Formulary includes the ingredient list and nutrient data for one serving size as provided by the manufacturers as of April 2010. Please refer to product labels and manufacturer websites for the most current ingredient and nutrient profiles.

Resources - The Resources section of the Nutrition Care Manual contains additional information for your practice.

  • The Cultural Food Practices section contains history, food practices, meal planning, and food dictionaries for many different populations, including Native American, Caribbean, Asian Indian, and Hispanic cultures.
  • The Equations section outlines the REE/EEE calculator equations and evidence-based guidelines for equations by condition.
  • Malnutrition Coding is where you can find notes on conducting physical examinations for malnutrition and the new Parameters Useful in the Assessment of Nutritional Status table, as well as updated information on malnutrition criteria and coding terms.
  • Easy access to the Dietary Guidelines for Americans (2015), The Academy of Nutrition and Dietetics Evidence Analysis Library, and the Nutrition Care Process.
